TERMS AND CONDITIONS

1. The following treatment is excluded from the agreement besides any discounted pricing per our price list :

  • Orthodontic
  • Treatment
  • Dental Implants
  • Necessary Medicines and Drugs for Home use
  • General Anaesthesia, IV sedation, Inhalation Sedation
  • Oral Hygiene Products and Aids
  • Treatment Provided on Referral to Another Dentist
  • Mouthguards
  • Out-of-hours Call-out fee

2. The membership has a minimum contract term when joining of 12 months from your initial start date. However, should a patient wish to cancel the monthly direct debit within 12 months of joining and subsequent annual term periods, you will be liable to pay for the difference of any complimentary check-ups, hygiene visits, and other discounted treatments that may have been provided to you as part of the membership within that annual term.

3. The Patient may terminate this agreement by giving at least 1 months’ written notice to the Dentist. The Dentist may terminate this agreement by giving 1 months’ written notice unless the patient has embarked on a course of treatment which will take more than one month to complete, in which case the agreement will terminate on completion of the treatment.

4. The monthly fee may be reviewed on a regular basis and is subject to an increase, due to unforeseen circumstances. The patient will be offered at least 1 months notice of any change to the membership monthly fees.

5. In the event of the monthly fee being unpaid for one month after it has become due, the Dentist may terminate the agreement by giving one month’s written notice to the Patient. The Patient will be liable for all sums due to the Dentist under this agreement until the date the agreement terminates that is two months after the payment was due.

6. The fee will be payable to the Dental Practice by monthly direct debit, administered by an authorized payment institution, regulated by the Financial Conduct Authority.

7. If the Patient fails to attend an appointment or fails to give 48 hours’ notice of cancellation, this appointment will count as an examination appointments available under the scheme.

8. This agreement is personal to the Dental Practice and the dentists practising there. Benefits cannot be transferred to treatment provided by any other dental practitioner or at any other practice.

9. Any disputes arising under this agreement, if they cannot be settled through the Practice complaints procedure, will be settled by arbitration.

10. All written notice should be sent by either recorded delivery post or by email to the last known address of the Dentist or Patient.

11. Stokenchurch Dental reserves the right to change these terms and conditions at any time without prior notice. In the event that any changes are made, the revised terms and conditions shall be posted on this website immediately. Please check the latest information posted herein to inform yourself of any changes
